Background Screening Requirement

In order to be a volunteer or employee with Community Wellness Center, each candidate is required to have a criminal background check completed prior to beginning their service.

By selecting the appropriate link below, you will ensure that the
correct type of background check is conducted on your behalf.

Once your background check is complete, the results will be made available to Community Wellness Center for their review.

Volunteer candidates will be required to self-pay for their background check.
